SNF Compliance Hub is a product of EXE, LLC ("SNF Compliance Hub," "we," "us," or "our"). This Privacy Policy explains how we collect, use, and protect information when you use snfcompliancehub.com, app.snfcompliancehub.com, and our mobile applications (together, the "Service"), as well as in our sales and support interactions with you. By using the Service, you agree to this policy.

Information we collect

• Account information — your name, email address, role, and sign-in credentials (a password or a passkey; passkey private keys never leave your device).
• Facility information — the facility you associate with your account, including its CMS Certification Number (CCN) and state.
• Content you provide — self-audit answers, notes, photos, deficiency text or scanned documents, incident descriptions, and drafts you save.
• Usage and device information — app version, device type, and diagnostic logs used to keep the Service reliable and secure.
• Information collected automatically — like most websites, we receive standard log and device data such as your IP address, browser type, pages viewed, referring page, and timestamps, through cookies and similar technologies.
• Information you provide to us directly — when you fill out a form, request a walkthrough, purchase Pro access, or contact us, we collect details such as your name, organization, email address, role, and anything you include in your message.

How we use information

• To provide the Service — including generating checkups, F-Tag guidance, and document drafts.
• To operate AI-assisted features (see below).
• To provide support and respond to your requests.
• To secure the Service and prevent abuse.
• To improve the Service, using aggregated or de-identified information where practical.
• To communicate with you about SNF Compliance Hub, including product updates and, where you have opted in or where permitted by law, occasional informational messages. You can opt out of non-essential messages at any time.
• To comply with our legal obligations and enforce our agreements.

AI-assisted features

Some features — such as Plan of Correction drafting and Incident Assist — send the content you submit to our AI service providers to generate a draft. Drafts are labeled as AI-generated and are intended for your review. Please do not include resident-identifiable information beyond what is necessary; the Service is designed to work with de-identified descriptions.

Public data sources

Facility profiles, star ratings, and survey history shown in the Service are drawn from public CMS sources, including Medicare Care Compare. This data is presented as published by CMS and may lag your facility's current status.

How we share information

• Service providers — hosting, AI processing, and email providers who process data on our behalf under contract.
• Legal requirements — when required by law or to protect the rights, safety, or property of our users or others.
• Business transfers — if we are involved in a merger, acquisition, or sale of assets, with notice to you.

We do not sell your personal information.

Security

All traffic to the Service is encrypted in transit with TLS. We support phishing-resistant passkey sign-in, scope accounts to a single facility, and limit internal access to production data.

Retention and deletion

We retain your information while your account is active. You can delete your account from within the app at any time, which removes your personal information from our production systems within 30 days, except where retention is required by law.

Your choices

• Access, correct, or delete your account information from the app's profile settings.
• Contact us to exercise privacy rights available in your jurisdiction.

Children

The Service is intended for use by professionals and is not directed to children under 16. We do not knowingly collect information from children.

Changes to this policy

We may update this policy from time to time. Material changes will be announced in the app or by email, and the "Last updated" date above will change.
